ok, thought for sure that would do it but still not working correct. Let me tell you what I did and see if you can see an error.
After I did File/Export then Manual Server Setup, then OK. In the Form Exported window, I copied the code from that window into the file where I want the form to appear and saved that file. Then I clicked on the Open Explorer button and I saw a folder with the name of my form and also that same name with a php extension. I copied both the folder and the form and pasted them into the same folder where the file containing my form is located. Uploaded all three.

Hi Janice what program are you using to create the page the form is to go into. Let us take it one step at a time. You are possibly missing a simple step. That happens. If you get really frustrated I will help you get one form working for you. After that you should have no trouble.
Use the contact form in my signature. I just finished showing a young fellow how to use another of CC's software. So my assistance schedule is open.
