If you're on Version 8, and it's a Coffeecup application, I think you're using the old "Classic form builder" which is no longer being developed or supported.
Download a trial of Web Form Builder v2.3 and you'll see the method Soren is describing. Then, if you are a registered owner of the classic form builder, get in touch with Scott or Suzanne as they might be able to offer you a sweet upgrade deal.
On the other hand, if you are happy with the old software for any reason, it can also do what you are asking. In v8.x Just click the button icon to add a new button and in the settings on the right pane select button type as Upload. Then, in "Configure Form Settings" you need to enable the file types allowed and set if they are to be included in the e-mail notice you receive or stored on your server, or both.
