Turning form submission into PDF -...

First and foremost..I am not a web tech person. I was asked to make a form for the office website by my boss. So here I am.
I made a form for patients (for a dental office) to fill out online to be submitted and printed off by office staff. My problem is that when the form is submitted, it comes in a very basic line by line format that doesn't print off well.
I would like one of two things to happen. Either have the form come as PDF so that it prints nicely or have the form come looking exactly like the form I created, see attachment.
I used this program because after some research, it was the easiest way to have a form on a website without having to buy storage each month ($50 for 500 submissions kind of thing).

The request is either to have the form come as PDF or to have the form come looking exactly like the form created.

I think it might be worth considering a third option: using the mail-merge facilities of word processor and desktop publisher programs. They can be used for printing out CSV fields in any layout one desires.

Hi Kaitlin,

I use Responsive Email Designer in conjunction with Webform Builder. This will easily allow you to customize the layout of the email you receive.
