Open your form as if you were going to edit it. Now save it using the File menu and choose "Save As" and save it to your desktop.
After you do that you should see a file on your desktop with whatever name you gave your form and it will end in a .fb (if you have file extensions turned on on your computer.)
If you only see a file with your form name there and it doesn't have an .fb at the end of it then that is most likely it, now send that file to CC. That is the file they need, the file your form actually is saved as, not the exported stuff. The file you open to edit your form.

I did what you said, and it did put a icon on my desktop.
In my desktop files I only see the file with .fb

Ok, I loaded the file in my httpdocs files on my server.

The reason I can't send a .fb file is you do not allow that extension.......

You can send any sort of file by encapsulating it in a .zip archive. .ZIP is one of the formats listed in the error message you cite. Many zip / unzip utilities available for MS-Windows are listed at http://download.cnet.com/s/zip/ (I can't comment on one or another of these because I use Linux, which has its own set of alternatives.)

You do understand the concept of uploading right? You do this daily anytime you make a change to your website or even upload this form to your server. It is called an FTP client. What extra software is required then? You already have what you need because you are using one of those tools now. Use that FTP client and upload that file to your server.
The file I have asked for (you posted a link to ST-ContactForm.fb) so you know what we needed, but that file is not there. Again, this is the file we need not some PHP file, folder, document, pdf, graphic or html page. Just the .fb file, only the .fb and nothing but the .fb file. If you copy and paste any text then that is not the .fb file. Again, just the .fb file.
