I have created the below form online but it doesn’t look professional enough
for me.
I was checking this software but I am not sure if its capable of producing a
better form, note the language option I have on the top of my form.

I am looking for exactly the same when it comes to contents and functionality, but professional.

Hi, I see nobody has answered your request.
The form software you have used is unknown, at least to me, CoffeeCup has its own form building software, Web Form Builder (WFB for short). But I think it is not able to do the calculations that you need, unless our form expert, Eric, has a solution for that.

I tried entering some items into your order form, but the sum at the bottom did not get updated, it stayed at 0.00. I know of some form software that does calculations (if Eric is not able to help you, that is). It's called Simfatic forms. I have used it for simple stuff in the past, and the support is quite good - and quick.

I was really trying to figure out what they want as far as professional. Kind of a broad term.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

I agree, 'professional' is a wide term. Maybe it would look good if the form were not all that long? Maybe make it in columns? But as I said above, it doesn't seem to do the sums necessary, the bottom line stays at zero.

Some conditionals to show some of the content based on some choices would work. I'll take a look at the math.
