Are you uploading these files yourself through a private ftp, or are you using the built in ftp s-drive? Because to me it looks like your missing a lot of files.. not the coding..

Looks to be that your regular expressions are all set to require 0-9

After I made the form in The Web Form Builder Lite I used the build-in publish button and it started uploading to the S-drive and then generated the code that I've used...that's all. It's late here in Denmark now, so I'll get on it tomorrow again. Thanks all for your kind help.
