Came up with a fun way of using Web Form Designer as a way to include a ratings system on a testimonial/review page I've done for one of my colleagues - a veterinarian. Here's the "test" form:

It works really well on default theme - if you want to change the form theme, you will have to simply retweak the font color to make sure it blends into the background and tweak the image padding to line up the radio buttons with the image lines of stars.
