I have used a lot of FTP software over the years but Coffeecups Direct FTP allows you to view permissions for folders and files and your website. You need to have a set of permissions that will help prevent hackers finding vulnerable folders.
If you permissions are incorrectly set then this gobaloons can get in quite easliy and do their thing. Although the ISP should be keeping an eye on folder permissions.
Also, if passwords are too easy to guess then they can work be guessed. Maybe the hacker is someone known and guessed or knows the password. All the other suggestions and courses of action previously mentioned are great to take on board.
One can never be too careful, no matter how time consuming and counter productive it may seem - until your hacked!
