I am typing in an url for an image element and it always shows up as a coffee cup.

...also, when using multiple choice, I have to keep typing in the option names repeatedly since it consistently changes back to the default field name.

The URL you are typing is for where you will go if you click on the image. To change the image, click the + icon to the right of image_default.jpg

When I use multiple choice questions, I always have to add the option information multiple times as only some or even none of it is saved to the form. Unfortunately I am doing a 25 question multiple choice form and it is taking me forever since I have to keep re-entering the options for the questions. They just revert back to the default randomly.

OMG, some of the option fields just won't hold the change. I don't know if I will ever get the fields all in correctly. Only 9 of the 25 questions are holding the information... I keep typing them over and over and over again...

Not sure which element you are referring to here. We do not have a multiple choice questions element. But I would recommend you delete the element and readd it. This should clear up the issue you are having.
I'm referring to the radio buttons that allow for multiple possible choices.
It's just flaky and I am powering through it. Hopefully I am catching all of the ones that have reverted to default. Entering some of them endless numbers of times...

For radio buttons each group has to have it's own name. So if you copy and paste the radio button group the form builder will assign a new name.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
