I would reexport the form try reuploading the form files.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Well, I deleted all old files on the server
Downloaded all new files from FormBuilder
Uploaded all new files to my hosting server

Now, the form has no outline, no order and is a jumbled mess.

Yes I see where it did not upload. I uploaded it again, with images.
Cleared Cache,
Got out of Mozilla
Got back into my dashboard
Refreshed my page (with form)
But again, when I view it, no CSS, no Captcha, no order.

Okay, I had to recreate the default folder for the css and images (honest it was there before!) :-)

The form shows up, but not the captcha
