It works, just needs a little fine tuning. I just tried it.

Oh, I just had to tell it to return people to the main page after submission.

Yes, thanks. But as I posted in the suggestions group, there's a logical dilemma there. The radio buttons need to be allowed to start off totally unchecked, and set up an OR logical situation between them and the fill boxes, where either a radio button can be selected OR a fill box. It's in the referral section:
