My suggestion now is to install Wamp and save export the form files to a folder under www use Localhost to run the form html file and if the form structure is correct it will save data to the storage/csv folder.I test all forms this way, makes it easy. I do not use Wamp's SMTP I use a gmail account for that when the form is working and online. It is amazing how simple errors are overlooked. I placed a comma instead of a full stop in code and it broke.
I keep thinking that one form you made and this one didn't that points to something you have done my friend. It happens to all of us. How about giving out the fb form less your details obviously, I am sure we can get I sorted for you
