The reason for the second csv file would mean one already existed under a previous form version so that was archived and a new one created.

Publishing to S-Drive takes about 10 seconds as you only need to upload a few configuration files. All the PHP scripts are on our servers already.

The fact you have the ability to change permissions should mean you have ftp rights. Get a ftp program. following on from what Eric said. just try ftping some files up to your server to check they are accepted. Then if that works. Delete the form files online and re instate them.

Work on the assumption that the software is not the culprit. It does require that the form settings are correct though.

Make sure you are uploading the correct folder and the accompanying php file. warnings are not always totally fault related and can be triggered by other factors.
