I had to delete my post above because in the haste I had assembled some wrong files for that form. I'm working on it now and will post again in a bit...

So here we go again. I hope the files are correct now.
If you want to use it, make sure you pop the two html pages and the contact.php into the same folder.

After testing, this works well for me and the form layout and responsiveness is very attractive. Two minor points :-

1. I have been unable to prove this works from a local server.
2. The phone number is not a required field but subject is and the asterisks in the form layout require update.
