What software are u using for the form?

It is probably SD.

Tony, you need a back end script. You can either
Create the form with Web Form Builder (WFB), which has everything needed
or just use the back end scripts produced by WFB with your SD form
or visit my sharing folder and use the form I have, including the script. See my singature for URL.
