At the bottom of this page there is a form
http://129.121.16.205/~unique/
How was it constructed? I don't think I can do a form like this in CC Form builder and the client wants it to look just like this one. Any tutorials on setting this up?

You can build the form within RSD with the inputs and styling as needed. Then you add your formbuilder files dropping the styling. It may take a fe adjustments depending on validation.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
