Slider removal form pasted html code

I am unable to eliminate the vertical and horizontal slider controls on the website that I pasted HTML code. Any suggestions?

My guess is that your form size width exceeds the width of your website page. Try adjusting the website page width or the width of your form (form would be the easiest as there's really no need for it to be as wide as it is. That should fix any horizontal scroll bar showing up. The vertical one is probably because you have altered the code to try accommodating this? If not let us know. There shouldn't be a vertical scroll bar if you've setup the form correctly and pasted the code as it was created for you.
