I need to create an intake form with numerous fields, and be able to populate the data on to a Word document or PDF from the submit button. The user will fill out the fields which will cover half the fields on the Word doc, then print or email it as an attachment for someone to print and fill out the rest?

Can this be done?

The best approach will be to build your own myforname.php to process the data and convert your data to the needed format. This will be a one off custom script and can't be done within the application itself.
