I just created my first form, it is here:

I would like the information that is returned to be formatted in a similar way, so that I can print it out on one or two pages and be able to file it, review it and refer to it while I am conversing with my patient.
What is returned on the S-Drive is ... adequate... but not exactly user-friendly.
What is the best way to go about getting my processed forms to look as professional as the front-end form?

Formatting Processed forms?
That is actually something you have full control of regardless of where your form is hosted. Simply go to the Settings > Email Notices > Configure Email Message. Here you can adjust the format to look any way you like.
