Hi there,

My web form seems to be working fine unless I ask it to send a notification email. Without checking the notification email box, it submits the data to MySQL just fine. This is the error message that I get:

Your form could not be submitted for the following reason(s):
Failed to send an email to "customerservice@physicianlitigationsupport.com".

My domain is physicianslitigationsupport.com

Here's the email notification control panel in web form builder (attached)



Any ideas what's wrong?
