URL in which the form is placed within:

URL in which the form itself is located:
http://accidentinjuryattorneyhotline.com/contact_form.html
(viewed source as instructed in help .pdf file, did not see any object code, but mostly script codes since used PHP option and not ASP, I assume.

Hosting Provider is Yahoo, and they use a Linux Based Server with a version of 4.2.11 PHP. Simple email form with option of send results to email (no text or database, as want to keep it simple). Went so Hosting Control Panel, and PHP/PEARL and changed default email to match that of the configuration settings of To: for email delivery. Use Dreamweaver CS4, just simply copied code (variable form data) and paste in page, to where it show form object only, but the form itself when viewed in browser (IE v8 I think), error repeats everytime, and not sure what I did wrong as data input was correct when testing.

Error received states:

I had the same problem when I was loading the form on a school site. www.ecolemdhouet.com - Contact Us page...

While the site name is a .com address, the files are loaded on the parent site for Calgary Catholic Schools - "www.cssd.ab.ca" Their mail server for cssd.ab.ca will not forward mail to non-district email addresses.

So, unless the mail address is "somename@cssd.ab.ca" the "unable to send mail" error pops up.

You may want to try the form useability with an email address relative to "accidentinjuryattorneyhotline.com" or check your mail server settings for mail forwarding. :)

Good Luck!
