I purchase a Clean Marine - HTML Editor Theme template that has an email form on the "contact us" page. How can I make the email form functional? The code is asking for a formmail script.

I'm guessing you need a php form mail script that the page uses sends the data to in order for the server to process the email request. The theme did not include this script? There are plenty of free php email scripts available on the web that you can tweak to work with your form.

you could replace it with a CC form builder form?

Is there a program that will allow me to create a web form with an if-then feature.

For example, I want to ask visitors if they are male or female, then have an additional drop down appear asking more questions based on their response. Is this currently possible?

CoffeeCup Form Builder does not offer this functionality. You would probably have to have a form script custom coded for you.
