I have tested out my form that I created using Form Builder. The results are supposed to be sent to my e-mail address, but I haven't received anything. In the form settings box I specified that I wanted the results sent to my e-mail address.
Any ideas?

TIA,
Annette

Can you please provide a link to the form? Someone might pick up a clue from the code.
Have you checked your spam/junk email folder? It could have been filtered.

Yes, I've checked my spam folder. Nothing there. I*did* receive e-mail to another e-mail address, however. But only one e-mail and I've tried numerous times.

Here is the link: http://www.removetypos.com/UploadFileForm.html

I would really appreciate you taking a look at it.

Thanks.

I see now what's happening. The form is being sent to the e-mail address *entered into the form*. Why?

Also it is not attaching the file that I am specifying.

Thanks for help.

Could be several reasons, but most likely you forgot to put a check mark somewhere in the settings. Your form is set up to send an "auto reply" to the person who sent the form, so the email address entered into the form is the "auto reply" address, and they receive a copy of what they entered into the form. That part is working, so we know you are on the right track.
In the form settings, in the form results tab, make sure tht the "e-mail me the results" is selected, then make sure there is a check mark in the "e-mail form results to these addresses", then make sure to put the email address (yours) in the box beside "To". Save and upload.
If that does not work, switch your email address to the "CC:" field, save/upload and try again.

For attaching the file, your form is only set to be able to attach .doc and .pdf files. If you try to send any kind of image, it will not go. You can add more extentions in the "file upload" tab. Be specific, such as .jpg and .JPG in case the sender has CAPS for the extension.

Try that out, let us know if we made any progress

What server/host are you using? Could it be Yahoo?

I've checked everything you mentioned, put in the e-mail address for both "To" and "CC". Saved, uploaded, and NO CHANGE.

I specifically only included .doc and .pdf files. The file I am sending *is* a .doc file.

I'm using lunarpages as my host.

Thanks again - any add'l help would be much appreciated. I'm stumped!

I think Bruce deals with lunarpages, and it has a few quirks. Try taking out the "To:" field. Leave it blank an only have the CC: field with your email addy. See if that makes any difference.
Also try this....in your form, where you want the email address typed in, remove the email verification, meaning uncheck the checkmark for "email address field". Save, upload all files and try again. There has to be something that is not letting it email you.

http://nic-nacs-plus.com/web/F2.jpg
as you can see I recieve to both addresses
How are you uploading the form? through VSD or through the form program? If VSD is the WWW folder selected as /public_html?
are al the files located in the same place together?
I just made a form and used the verification of email and it worked fine for me, i also unchecked the email address field because I was having a copy sent to my Yahoo email.and with attchments,Oh and I used VSD as well. here's my test form

I have tried everything you have both suggested - to no avail! Maybe I'm doing something else wrong?

I am making the changes in Form Builder. Then I save them, upload all the files to the public_html directory.

Then I go into VSD, delete the old form, add the new one. Then I save and upload it to the web.

I just don't know what else to do!

BTW I am using a trial version. That shouldn't matter, should it? Except that if I can't get this work, I won't be buying this software.

TIA.

Hi Tia,
if you havent already, have you tried a template form? and just adding a upload button?
I tried the trial version and it seemed to be ,so unless your playing in the code, there's no reason for it not to work. Here's how I placed it in VSD.

and here's the form

give it a shot if you have the time. these are all on Lunarpages server
