i need a one page with video and an opt in box..

Not to sound silly here, but have you tried any programs like the Visual Site Designer that might be able to help you design, control and maintain the site yourself?
I'm not sure that it can do any kind of "opt in box", but you can get a free trial period of any of the CoffeeCup software from their website. Could be a low cost alternative to hiring a designer to build it for you....just a thought.

What about adding a small form from the Web Form Builder with Name field, Email field, and submit button? The data can be sent to a MySQL database.

Or a button that links to a full page form with lots of fields.

I used to use PHPBB until I found SMF Forum.
