Can not integrate the form manually into my website....tried to upload the two files in the themes directory for wordpress and then copied and pasted the html code on to the page I want the form to appear...but keep getting error....read a lot of articles on iframe, and all this other stuff, but just can not figure it out...works great with s-drive...but you only get one form before you have to purchase...so if anyone has some sort of tutorial, that would be great!.... thanks

Make sure you have the folder with the files in it, and the .php file all inside the location that the page is on. Usually this would end up being the root for a lot of CMS type sites like Wordpress and Joomla etc. Also make sure that you are uploading only the contents of the saved folder (the folder and the .php file inside the folder the program creates.
