My first thought was to do 5 images, and then 5 radio buttons. But they don't line up well, and I feel like there is probably a better way.

Anyone have an idea?

This is a tricky setup to achieve. You'll need to add custom styling by either adding it to the form builder or hack it later. I did a quick example to get you close.
