I caught some spelling issues.:lol:
Here's my screen shots.
First is what the user gets after submission and the second is what it looks after clicking reply.
Maybe I'm not getting what your trying to say but I don't see any form name in any of the text in my email.:)

It looks like you're replying as the customer to their notification after they receive the thank you, rather than replying as the owner of the form to the emailed notification that someone has used your form. Do it the other way and you should see a fake email address of disclaimer@aol.com in the body of the emailed reply to your customer.

Also, for the test it would be good to use a fake customer email address. Since it's not needed in the test, it doesn't matter if the customer email address is fictitious since we're testing on the owner's end of things. Using both the owner email as the customer email gets a bit confusing for this test.

Eric,
Your submission.gif looks like an auto-response that's sent to the person who filled in the form.
What about the initial, main message to you, as the webpage owner, from the person who filled in the form?

OK, Now I know what you want me to try. That wasn't how I was interpreting your post. I'll give it a try and let you know.

It didn't dawn on me with your first images, but even with these, your reply email's body should have an email address connected with your domain name. Since your domain name isn't AOL.com can you do this again using a different email address in FB, since AOL is in a world of its own?

I set the form up with my work Email and it looks pretty much the same in Outlook with no form name in it anywhere.:)

Good, but name the form something besides your full name. Thanks for going through this trouble.
