This is the message I received:

Warning: [ err 2/line 248D:\Sites\fastsale-uk.co.uk\public_html\fastsale-uk\fbapp\php\mailer.cls.php] mail() [<a href='function.mail'>function.mail</a>]: SMTP server response: 501 5.5.4 Invalid Address

What can cause this?

Server test looks ok but the mail part is different then I've ever seen.:)

From what I can see, is it possible the form has been loaded from a localhost and from there it doesn't go out?

Yes, I built for a client. All his site is built with ASP.
I just realized it's in a frame.
Maybe you might take a look.

Forget it for now. I've put back my client's old form for now... until there's a solution for ASP server side.

I can't get pass the primary phone number even when I enter the exact phone number you have as a prefill.:/

this is the error I get when I submit my simple little form, also the same error on a more complex form:

Your form could not be submitted for the following reason(s):
•Warning: [ err 2/line 396/config.cls.php] session_start() [<a href='function.session-start'>function.session-start</a>]: open(/var/php_sessions/sess_c7e4ba425a6edd5155e83f0dca1308db, O_RDWR) failed: No such file or directory (2)

any one else run into this?

Sounds like PHP Sessions are not enabled on your server. Once you enable that, the form should start working.
