I've built and published a form in a temporary location and it worked fine. When I loaded it to it's permanent location suddenly the form is not showing even though I put the exact same files in place.

I'm wondering if the problem might be the version of PHP the permanent location is using. Could that prevent the form from showing?

The problem there is your server is running a very old version of PHP (probably 4). You need to have PHP 5.3 or higher.
