Embedding the form code into a page and the fields displays 100%

But when submitting I keep on getting this error (tried to submit it to an external confirmation page as well as the one generated by the form designed.

Also tried a clean test.htm on the server and embedded the form code into it - a simple page with just an html tag body tag and the form code. I suspect it could be the server settings but not sure what.
