Was reading on the forums and saw that you can put more than one form on a page. I made individual forms and the forms show up but one is not like I designed it at all.

The one thing to try is to remove the spaces from the form name. You should never use spaces in any directory or file names on the web.

If you click on Immigration the rest shows. You must have a conditional set.
