Show Element not working on iframe...

Not sure if I did it right but I built a form:

http://healthfirst.coffeecup.com/forms/trainingform/

And I tried to do the iframe thing to incorporate it into my site:

http://www.healthfirst.com/iframe-test.html

But what's happening is all of the conditional elements that should only show depending on what people put on the form are showing up on the whole form; nothing is hidden like on the coffeecup version. Not sure what I'm doing wrong...

The script code is correct and the file references (css and js) are all correct.

This is a site hosted in an Adobe Business Catalyst environment and I know it does not always play nice with other scripts. Do folks have any ideas what I'm doing wrong, if anything?

Did you use the embed script? :) You must have the js missing.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Yup; I used the JS script; but Adobe Business catalyst won't let you run certain scripts on the site and that's pretty much certain what's happening here.

I actually had this whole thing figured out on my own simply using my own form and zero JS etc; but there were so many variables in the answers that my head started to hurt; that's why I went looking for a tool. This is clearly an awesome one that let me build the form very quickly; but of course that dang Adobe Business Catalyst (otherwise known as just about the worst e-commerce platform ever, IMHO) just screws things up enough to prevent you from doing what you really want....

Do you have the ability to place the files in a different server? Does Adobe have a storage folder? This way you could use an iframe in your page.
I can't hear what I'm looking at.
