TWO MAJOR POINTS before you go any further (and thanks, Scott, for pointing these out) -
1. Make sure you're using the latest version of PHP;
2. Make sure you've downloaded the latest version of Web Form Builder (in my case, 3008)

I wanted to incorporate a Coffee Cup Form into an existing web page, rather than just use a Web Form on its own page. This is how I did it -

Very helpful Phil! Thanks! I have to deal with multiple forms and am having trouble getting the pathing right under my root directory.

BTW, is it standard where you are to have the remote directory in FTP on the left and local on the right? In the US it's the other way around.
