I took a look at your form here at http://tlawfirm.com/Tenenbaum-small-for … ertest.php does not have cURL libraries installed. Without that, the form will not function. Contact your hosting provider and they will be able to upgrade the PHP installation for you. Once upgraded, your form should start working.
