Are you saving your data in any database on your server. The only thing that the server test shows is the PDO - SQLite driver: Not Installed

I'm not sure if this will cause this error or not.

No, I am not saving the data. It looked as though it was trying to write an error log and failed due to some permission issue. But I am unclear as how to handle that. I am removing the email that is causing the error but not sure how to tackle the inability if some other error is generated.

Mark what I would do. Create a page and add that form to it and test it. If that works OK Then add one of the scripts and test that. Continue until you have added all of the page elements. You have a lot going on. It may not be the form that is causing the problem. This way you will find out which of the elements to blame. It does seem to be invoking some of the forms scripts.

I put bad email address back in, tested from /contact/contact.html page and same issue occured. I removed bad email address after this test.

I'm getting the same error, sent an email to support, but no one has answered my question. Why should it work only with a user had SD account?

What are you talking about! I replied to you on May 29th at 9:28pm. You never replied back to me! I may also add I replied within 3 minutes of you submitting your question too.

The problem is you are on a very old version of the software. Just update to the latest version and you should be fine.

Sorry, Scott. I was in a remote area and lost track of my emails. I'll try to uninstall the old version and upgrad to the new one and them see the results.

I just download and installed the new version (Web Form Builder 2.0 Windows – Web-Form-Builder-Full-2.0.4206.exe, release on May 31, but still having the same problem. Not sure if the problem is on my end.
Even after I had unistalled the previous version, restarted the computer and then install the the latest version of Web Form.

When tested locally I got a message 'Form Submitted' but was not redirect to the choosen page

That is a permission issue on your server. To fix this, change the permissions of the Storage folder on your server to 755 and you will be good to go.
