Notification Mail shows some source code

The form its working good, but the notification and confirm mail and is showing some css source code.

How to prevent this code?

Looks like you used the wrong template for the email response. You have selected a shopping cart price template. I would just select the regular confirmation response and re-upload the entire form to your server.

Yes there is, the drop down at the bottom will give you a few choices... just make sure you select ones that are not cart based.
