Try reexporting your form and uploading it. I've had files not get exported or get corrupted.

I would double check your notification and confirmation email html in the form builder to make sure something different didn't get selected from what you were using.

I reported that months ago. Never had reply or solution, it works fine if I export it to Sdrive but manually exporting as Wannab web says the notification/ confirmation just html, I tried all sorts of stuff but to no avail.
