I got it, and there are a whole lot of things that are wrong, that not only cause the form not to display, but also stop browsers from rendering the pages (all of them that you have the Google Analytics thingy in) properly. One of the things that's causing some errors is the same as the HCP website I helped you with a couple of years ago, (fortunately Not as many pages as the HCP one. which if you remember took almost 3 weeks to rectify)

Can you confirm the version of "Site Designer" you are using please

When you do a manual export of the form, WFB generates a folder containing everything needed to make that form work, and a php file that opens the folder, they need inserting into your site designer project root folder in your resources thingy. You haven't done that, which is why the form doesn't show,
There are other things that need correcting in the site designer project,
Can you therefore also send me the WFB project file so I can put the required folder and files into your SITE DESIGNER project, I'm having a bit of trouble with my video screen recorder which I hope I can fix, so I can do a video showing the process.

Hi Byron,
The error log looks like youre using an older version of form builder. What version are you using?
Try uploading your files again as they don't seem to be there.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
