Wow!! That's the answer? With the software I purchased I am only allowed to upload one free form to the S-Drive. I had no intention on paying to upload more; otherwise I would not have purchased the software.

Have you tried changing the Settings of the form?

Go to Actions menu then to Settings then click the second tab named E-Mail notices.

Change the E-Mail address in your form to match the one you wish the form results to go too...

Save, export and REPLACE the existing code in your HTML with the new script that is given.

Erase ALL files of the old form from your server.

Upload ALL new files for your new form...

Are you logging to MySQL or CSV in your server? If so, disable that and see if it helps. I would also change the TO address to something not on your server like a yhaoo or gmail address and see if it arrives. If it does, then you know the problem is with your hosting provider and you can contact them to see where the email is going to.

Problem resolved!! This may help a whole lot of people. I actually did a live chat with my hosting company which always give excellent support. Anyway, somehow a email address was created with the name of the form I created as the prefix to the email domain that I put in the from field!! How did that happen is a mystery to me! Don't know if this happens automatically through Coffecup. Anyway, I was instructed to create an email address as the one that it was showing as the from address. As soon as I did that everything worked like a charm. My only question is, if I create other forms with different names will new email addresses be created for them? Maybe someone can enlighten me because I am not that savvy with this stuff but know enough to get around.:P

That was your host that did that.. Form Builder only knows and does what you tell it to do...

If you tell it something that is what it does... It can not think, it has no feelings. If spits out what you put in. :D

I would go back to your host and ask them why that E-Mail address was created.

They say the same thing CC says. The program can't create email addresses. It wasn't even set up in my C-Panel, they were just able to see in the mail log what address it was attempting to send to. At this point I could care less if man or machine is creating the email addresses. My forms are working and I know where to go to find out the names of these phantom email addresses and then create them in my C-Pane.

You just answered your own question...

You said... "they were just able to see in the mail log what address it was attempting to send to."

Which means the E-Mail address put into the settings of your form was incorrect. And that is something that you add in yourself.

Again I would check your E-Mail setting for the form...

Actually I have this issue happening to me too. Every time I test my form I get 3 email responses. One of them is the reply that I would get if I were a user of the form. One of them is the results of the email I would get as the TO: email address, and one of them I have no idea where it's coming from lol. It tells me that someone has filled out a form on my site and here is what they filled out. Gives me the results again and it's coming from the domain itself or something. Doesn't even have a .com on the end of it since the site isn't connected to the domain name yet. I'm working on it via the IP address yet so the email address is one created from somewhere that I didn't create
namecontact@IP address here

Where name is the name of the site, contact is made up on it's own and of course the IP is the ip of the site without a domain attached. I'm sure if the domain were linked to the site it would be:

namecontact@domain .com

Very strange, never saw this before. Something created this email somewhere, how do I track this down?

At the very end of this email it says:

Well to put to rest this issue (hopefully) you can go to the articles link in the support tab of CoffeeCup and read article ID: 936 "Overriding the default "from:" address.... So, just as I suspected the form builder software is creating these email addresses based on the name of your form and your domain name. That was pretty obvious. Now, I will look into inserting the workaround they suggest so it can use the email address I want not the ones it creates.
