Did you get it figured out? I can see the whole form, it just has a scroll bar.
I like the form at the bottom better... Maybe your link could jump to it instead of using a pop-up?

From a responsive point of view, popups are not responsive and should be avoided when possible. Directly embed your form into your page instead.
