                          This form works, it was one I made a while ago.

Remove the spaces from the form name.
 It's always a good idea to keep spaces out of code. Use under scores or dashes if you want some separation for readability.

                          Got it to show up but now it wont submit.  It gives the following error.
Your form could not be submitted for the following reason(s):
Failed to open database [2013] SQLSTATE[HY000] [2013] Lost connection to MySQL server at 'reading initial communication packet', system error: 111.

                          Looks like some files might be missing.

                          That file is there, I can access it through the file manager on my server.  I can even click test on it and it says, cool, sessions are usable.

                          I don't think the form be responsive had anything to do with it. I think a few files didn't make it to the server.
