Can we see a link to the bad form?
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Try renaming the page your form iframe code is on.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

I renamed the page, re-uploaded the form and got this response...

Your form could not be submitted for the following reason(s):
Failed to open database [1044] SQLSTATE[HY000] [1044] Access denied for user 'thames_dogbender'@'185.181.124.16' to database 'thames_forms'.

Good suggestion but with no luck.

On the form that doesn't work try entering the same table name that works with the other forms.
I can't hear what I'm looking at.

I deleted the tables of the two working forms and upload the one that is not working. It still did not work but when I uploaded the two working forms, they are both working again. Renaming the page did not work so I don't have a clue.

Try creating a new database and a new user and setting their permissions when you do that. Then change the broken form to submit data to the database with the new user.

Thats another good suggestion but I have already tried that with the same result.

It's got to be something in the form that is causing the problem but I have no idea what unless its to do with the number of columns.

In format specific options, it says NONE but there is a dropdown list. Would that be causing an issue with this third form?
