I have checked and compared between the working form and the two forms that are not and I see that all permissions are the same.
I have read, checked and tried previous threads on this subject but the problem still persists.
Support by hosting provider is able to access all three forms and can find no problem on their side.

It's done on the server when you add a user to a database.
Are your two forms using the same database but different tables?

I have created a database for each form as the information has to be purely for a particular form.
Each form has 755 privileges.

On the form your having trouble with change the database to the same one as the other form is using and enter a different table name and then test.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

All I have to do now is sort out my other databases as we have five forms on the website.

I use the same database for several of my forms and name the table for the form I'm using. This makes it much easier to do queries and such as I'm not hooking to a different database so the main logon stays the same.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Hi Eric,
Thanks for your help but after getting two forms working, I am having the same problem trying to add a third one.
I have checked and double checked both settings and permissions and apart from the table for the the third form which should be generated by Form Builder when I upload it, the completed form although submitted to us as normal, cannot connect to MySQL database.
Help again, Please,

What's different in the third form?
Is everything but the table name the exact same as the other forms?

Hi Eric.
Nothing is different other than the table name.
All database settings and permissions are identical.
The only thing I found was that the permissions for the submit button image was different so I tried the form again after changing it to 755 but with the same result when submitting the form.

Hi Peter,
As Eric suggested the easiest way is to have one DB with a unique table name for each form.
If you are using multiple DB's check that the user has been assigned to the DB's also check that the user has all privileges on the DB. This is all done in your CP
