I was indeed using my iphone. Checked again today with an android based half-size tablet, and it works fine. The form isn't scrolling on the iphone.

Hi Eric,
unless its a cache thing the form on that link is still broken on my iphone 5 and samsung note2.
Tried refreshing on the phones and still have the problem. Tried resizing in FF, IE, Chrome, Safari and comodo dragon on the desktop and all don't show the form correctly.

Is there another link now to check with?
