migrating form to auto generation -...

I have been using web form builder to generate an order form for each new customer. It is a simple form but does have some jqueries in it. The only difference in the form when we create it from one customer to the next is a text field, the email address for the confirmation, and table name to post the data to - all readily available from a customer profile in a shopping cart database i already have. I wish to generate the forms automatically when customer signs up which my programmer can do for me. We put each forms in its own unprotected directory (no log in required - there is no payment functionality needed-just data entry).
so question....I like the form we have created. How do I give he programmer the form as a template to use but without all non-used the CoffeeCup files that come with it? hope this makes sense

sorry for the sloppy description of issue - I need to stop making hundreds of forms and automate the form creation upon customer signup and approval in my site. I do not want to require a customer to log in to access the form, so as a result, I think I need to place forms in unique directories. Need to automate their creation though

Hi Telimon,
Figure out which files aren't needed then remove them and zip up the files and email them to your developer or give them ftp access to the files online.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
