Check the two attached images. The first one is showing the tab Notifications. Underneath you can see what fields I have in the form, just Name, Email and Comments

So, settings-1:
To: your own address, to which you want to get the notification about a form being submitted.

CC: optional, if you want someone else to get the same notification.

Reply-To: Select, not type, the input field where your end user has typed his/her address. This is usually called email followed by a number, and the number reflects the sequence in which you added it to the form.

From: If you want to let your replies come from another address of yours, write it here. Otherwise use the same as above.

Email: Select a template. What you select will show in the Preview window.

Then, settings-2
To: Select the field holding the end-user's email address (Same as Reply-To above).

From: The one of your addresses that you want to send a reply from.

Subject: Can be changed by you, but is not necessary.

Email: Same as above, select a template and it can be previewed in Preview.

Also make sure you have an email element on your form.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
