including an image map in form

Hi,

I'm converting a printable form to an online version for a client. It is for a medical practice and the final question asks patients to mark an 'x' on the part of the body where they are having pain, symptoms, etc. Then there are line drawings of the front and back of a person. Is there a way to have this image on the form and let them simply click an area and have that sent in the form results, like with an image map?

Hi Cheryl,

I can't think of an easy way to accomplish what you want.

What I would try is, limit the "zones" on the body to as few as you can in practical terms and then make an image map where each zone links to a different form. The only difference between each form would be the criteria you see in the output, so each patient fills in the form the same way, but you know the area of their pain/complaint based on the origin of the form from your image map.

Maybe there is another way to do what you wish and hopefully so, let's see what others may suggest for you.

An easier way would be to use the HTML tool in the Web Form Builder to insert the images and label the image areas with letters or numbers rather than making it an image map. Then just include a checkbox or radio buttons or whatever type of selection tool you need and have them just check the boxes for the labeled areas that they coincide with. Much easier :)

Nice one Jo Ann,

That's why these forums are sooo cool. More than one piece of grey matter to rely upon.

Your suggestion is perfect, an image with regions/areas marked with letters, then a drop down menu, radio button list or check-box choice based on these letters. The user can easily click/check the right choice and you can quickly see at the receiving end which region they are complaining about.

Thanks. I had a similar idea after I made this point - to use checkboxes for predesignated regions. Thanks for the input!
