This issue I think here is caused by permissions not being properly set on your server. With an FTP client, locate the folder called storage and change the permissions for that folder and all folders under it to 755. That should then fix the issue.
If you still have the error after making those changes, try disabling CSV and MySQL logging if enabled and see if that helps.

Nice form for sure, good use of the program all the way around. I would make one suggestion though. Some of the choice setups you have for things should probably be radio buttons rather than check boxes. The reason for this is with check boxes both items can be checked on your form. With radio buttons you can set it up as "required" and it will not have a default answer chosen, and they will only be able to choose one then.
That's about the only thing I would change on the form, well done

Actually no.. I wanted check boxes instead of radio buttons, customers are supposed to select the options, it is the right fit for this forum, thanks though

Try deleting the form and re-uploading.
