You appear to be using the same types in the same order.

And is this a bad thing? :D
Text Field, Number Field, Text Field, Number Field, Text Field, Number Field....
I always figured the code reordered the csv/database output order according to the vertical down and left to right orientation of the elements, even if you dragged the elements around in the interface or inserted new elements. That's always seemed to be the way it worked in the past but I must have thrown it a curve ball.

I'm rebuilding right now. Made screen capture notes and I'm deleting all 6 problem elements - codes & fees. As I rebuild I'm first leaving the elements full width so they can't "flop around". Then after I'm finished with all the properties, I'll tweak the width and padding. We'll see if this does it.

BTW... and this should really go under tips I suppose, and may have been mentioned before...
But the way I work with new iterations of a working form:
I always create a master folder with the form builder projects for that client then I create new folders with the create dates of the form. As I rebuild the form, I can keep the title exactly the same so that any existing links I have don't get messed up, but I save the new iterations in a new folder and export the files there. I also make sure all my customization notes are
in text files clearly explained and copy those into each version folder. I'd go even crazier if I didn't do it this way.

I do something similar. I keep my forms in their exported folder and remove the _exported from the end. This way I can change the name of the form to a new version and alter the iframe code in the page to reflect that. This way if my current version has a problem I can go back to the old one easily.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

OK... redoing it fixed it. In addition to keeping them at full width until all parameters were entered, I also changed the width and padding values starting with the lowest element in the form first. This kept them from flip flopping.
Cheers.
