How do I create a subject line for an...

I have created a form that creates and email and it works well except that the subject line is a messy composite of
name, email address and some letters at the beginning of the message.
Should I just add a field for subject at the beginning? If so, how do I limit the subject line to only that entry?

Go to 'Settings' > 'E-mail Notices' > 'Subject'
See attached screenshot.
You may want to remove/modify some things in the body also, as high-lighted in the screenshot...

Also if you notice in Per's screenshot there's a dropdown list under the text box at the bottom. You can insert any of those fields into the subject line too so you can put a Name or email there to address to someone personally. :)
